Terms and Conditions & Privacy Policy

​

  • Before any appointments can be attended,  as a legal requirement of the Veterinary Surgeons Act 1966, an owners consent and veterinary consent forms must be signed. These can be obtained via the hairytailvetphysio.co.uk website, and upon request.

  • Hairytail Veterinary Physiotherapy reserves the rights to refuse treatment of any animal if deemed unsuitable to treat.

 

Hairytail Veterinary Physiotherapy Obligations

  • Signed appointment sheet will be provided following the appointment.

  • A post appointment a report will be provided to the owner and the vet within seven days of the appointment.

  • Hairytail Veterinary Physiotherapy will abide by the National Association of Veterinary Physiotherapists (NAVP) Code of Conduct.

  • Abide by the appropriate biosecurity measures and healthy safety regulations.

 

Cancellation Policy

 

  • Cancellations 24+ hours prior to appointment will not be charged.

  • Cancellations within 24 hours of appointment will incur 50% of the appointment fee.

  • Cancellations due to an emergency can be waivered at the discretion of Hairytail Veterinary Physiotherapy.

  • Cancellations must be made if the animal has a contagious disease.

Owner Obligations

  • The provision of the owner and veterinary consent forms prior to appointment.

  • Owners must report if there is a change, reduction in health and or at the request of the veterinary surgeon.

  • Post appointment the animal will require up to 48 hours off work or heavy exercise, please take this into consideration when booking an appointment.

  • Payment to Hairytail Veterinary Physiotherapy must be made on the day of the appointment. Treatment cost will be outlined in the confirmation email once the appointment has been booked. This can be done by cash or BACS. Sort Code: 77 – 02 -01. Account Number: 81805963.

  • Ensure appointment area is safe and the animal is prepared for treatment.

  • Treatment provided is at the owners risk.

Privacy Policy

The personal data that we collect about you will include data relating to your name, address and contact details, and the health/medical history of your animal. This data is collected as a way of identifying individual clients. All data will be stored securely and kept for up to 7 years (required for insurance purposes) and will be used by Hairytail Veterinary Physiotherapy for communication with you.  Any paper records will be stored securely in locked storage and electronic records will be stored on an external hard drive, locked securely when not in use. The data collected will be used as a record of your animal’s treatment and any changes that may occur in their health. You have the right to request access to your data, request corrections (if any errors in the data are identified) or deletion at any time. Information will be shared ONLY with your animal’s vet or other veterinary professionals involved in their care for the purposes of keeping them informed of progress and changes with therapy.
